SUMMER DAY CAMP 2026

Waiver, Release of Liability, and Hold Harmless Agreement (Electronic Signature Version)

 

Camp Schedule:

Each full week of June 2026 (Monday–Friday)

9:00 a.m. – 3:00 p.m.

Location: Mission Christian Academy, Fishers, Indiana

 

This electronic agreement must be completed and signed by a parent or legal guardian prior to a child’s participation in any Mission Christian Academy Summer Day Camp activities.

 

1. Assumption of Risk

I, the undersigned parent or legal guardian, understand that participation in Mission Christian Academy Summer Day Camp activities involves inherent risks. Activities may include, but are not limited to, indoor and outdoor recreation, sports, games, crafts, playground use, group activities, and general day camp programming.

 

I acknowledge that participation may involve risk of injury, illness, property loss, or other damages. I voluntarily and knowingly assume all risks, both known and unknown, associated with my child’s participation in the Summer Day Camp.

 

2. Release and Waiver of Liability

To the fullest extent permitted by Indiana law, I hereby release, waive, discharge, and covenant not to sue Mission Christian Academy, its board members, directors, officers, employees, volunteers, agents, and representatives (collectively, the “Released Parties”) from any and all liability, claims, demands, damages, costs, or causes of action arising out of or related to any loss, damage, injury, or illness that may be sustained by my child while participating in Summer Day Camp activities, whether caused by negligence or otherwise, except in cases of gross negligence or willful misconduct.

 

3. Hold Harmless and Indemnification

I agree to indemnify and hold harmless the Released Parties from and against any and all claims, actions, liabilities, damages, costs, and expenses (including reasonable attorney’s fees) arising out of or related to my child’s participation in the Summer Day Camp.

 

4. Medical Authorization and Emergency Care

I certify that my child is physically able to participate in camp activities. I acknowledge that it is my responsibility to inform Mission Christian Academy of any medical conditions, allergies, medications, or special needs prior to participation.

 

In the event of illness or injury, I authorize Mission Christian Academy staff to obtain or provide appropriate medical care for my child, including contacting emergency medical services if necessary. I understand that I am responsible for any and all medical expenses incurred.

 

5. Behavioral Expectations

I understand that campers are expected to follow all camp rules and demonstrate respectful and safe behavior. Mission Christian Academy reserves the right to dismiss a camper whose behavior is deemed unsafe, disruptive, or inappropriate. No refunds are guaranteed in the event of dismissal.

 

6. Transportation and Pick-Up

I understand that camp hours are 9:00 a.m. to 3:00 p.m., Monday through Friday. I agree to ensure timely drop-off and pick-up of my child. I acknowledge that only individuals authorized through the camp’s electronic registration system will be permitted to pick up my child, and identification may be required.

 

7. Photo and Media Release

Mission Christian Academy may photograph or video camp activities for educational, promotional, or school-related purposes, including use on the school website, printed materials, and social media.

 

By signing this agreement electronically, I grant permission for my child’s image or likeness to be used for these purposes unless I submit a written opt-out request to the school prior to the start of camp.

 

8. Electronic Signature and Acknowledgment

By electronically signing this document, I certify that:

- I am the parent or legal guardian of the participating child.

- I have read and understand this Waiver, Release of Liability, and Hold Harmless Agreement in its entirety.

- I agree to all terms and conditions stated herein.

- I understand that this electronic signature is legally binding and has the same force and effect as a handwritten signature.

 

I understand that my child may not participate in Mission Christian Academy Summer Day Camp without a completed electronic signature on this agreement.

 

This agreement shall be governed by and interpreted in accordance with the laws of the State of Indiana.
